From this page you can download forms and examples required by Financial Services.
If your browser is set up conventionally, you can click on the links below to save the file to disk. If you have difficulty with this, you should hold the Shift key down while you click on the link.
You should be presented with a Save As window which lets you choose the destination for the file. After you have saved the file to your disk, you should open it in Excel or Word as appropriate.
